What is CPS?
The Commonwealth Psychosocial Support Program (CPS) connects you with a non-clinical recovery coach who will support you to achieve your goals and get back to doing what you love.
The Marathon Health team will provide one-on-one coaching, and work in partnership with your family and carers (where possible).
This is not a clinical service, but a community-based support program. Each session with your recovery coach will be goal oriented in focus and delivered based on your personalised recovery plan.
Who is CPS for?
CPS is for people experiencing a severe mental illness that is impacting their ability to live the life they want to live. Participants must be 16 years of age and over.
Working with a recovery coach, you’ll learn strategies and develop skills that help you:
- Identify your strengths
- Connect to a social network that will improve your wellbeing
- Build resilience to achieve your goals
- Connect with family or your community
- Enjoy a full and vibrant life
- Build capacity to live independently in the community
Your psychosocial coach will support you to:
- Look after your mental and physical health
- Develop a recovery plan
- Achieve your goals through face-to-face and phone coaching
- Develop vocational skills and goals
- Manage substance use concerns
- Find and maintain a home
- Reduce your need for hospital services
- Test your eligibility for the NDIS
- Reach out to the people in your life who can support your recovery (such as carers or family members, your GP, services in the community and health professionals)
